The Seminole County Health Planning Partnership (SCHPP) began the MAPP process on May 20, 2003. The partnership has broad representation from the health care community in Seminole County and meets montly to accomplish the MAPP objectivevs and goals.

"Establish the Seminole County Health Planning Partnership to work with the Health Council of East Central Florida, Inc. to establish a local partnership to facilitate and coordinate need assessments in Seminole County and to implement a plann of action".

Seminole County MAPP Status
To date, the Seminole County Health Planning Partnership has completedthe four MAPP assessments.
On March 31, 2005, Seminole County will sponsor a health workshop to begin working on the identification of strategic issues and the formulation of goals and strategies.
The Seminole County Health Workship was a huge success and a generated productive dialogue between community partners. The workshop was instrumental in establishing the barriers to achieving zero disparity, the greatest concerns regarding unhealthy lifestyles, and the gaps in the current system of care. the group also identified the opportunities available within the community to effectively address these complex isues. As a result, three subcommittees have been formed and tasked with the development of an action plan. The three committees are as follows: System of Care/Access to Care, Healthy Lifestyle, and Health Disparity. The Seminole County Health Report Card has been completed. This looks at overall health as the result of a combination of personal behaviors, the community environment, and health care system policies and practices. The criteria have been identified and tracked for every state and rankings determined according to a proprietary calculation.
