Osceola MAP Process

The Health Council was charged with conducting the mobilizing for Action through Planning and Partnerships (MAPP)in Osceola County and to date has completed the Community Health Assessment. A community report card has been prepared and published using the health indicator guidelines set forth by the Health Council. Upcoming meetings include the Forces of Change Assessment and the Public Health System Performance Standard. Keep watching for limes and locations to be posted.

The Osceola County Health Report Card has been completed. This looks at overall healt as the result of a combination of behaviors, the community environment, and health care system policies and practices. The criteria have been identified and tracked for every state and rankings determined according to a proprietary calcuation.
