The Health Council of East Central Florida is conducting the MAPP process in each of our four counties.

 
East Central Florida Counties

MAPP is a community-wide strategic health planning tool for:

  • improving public health
  • developing an action oriented process
  • assisting communities in priortizing public healh issues
  • identify resources for addressing iterventions
  • taking action

The MAPP process was developed through a cooperative agreement between the National Association of County and City Health Official (NACCHO) and the Centers for Disease Control and Prevention (CDC).

The MAPP process brings four unique and comprehensive assessments together to drive the development of a community strategic plan.

  • The Community Health Status Assessment
    analyzes data about the health status, quality of life and risk factors in the community.
  • The Local Public Health System Assessment
    measures the capacity of the local health system to conduct essential public health services.
  • The Community Themes and Strengths Assessment
    identifies themes that interest and engage the community, perceptions about the quality of life, and community assets.
  • The Forces of Change Assessment
    identifies forces that are occuring, or will occur, that will affect the community of the local health system.

Upon completion of the assessments, participants review the results of the four MAPP assessments along with the vision and determine a proactive course by developing an ordered list ( Identifying Strategic Issues ) facing the community.

The Formulation of Goals and Strategies provides a connection between the current reality (where the community is now) and the vision (where the community wants to be). Together, the goals and strategies form a comprehensive picture of how the local public health system partners will achieve a healthy community.
Planning, Implementation and Evaluation are the three key activities that make up the Action Cycle. Each of these activities builds upon the others in a continuous and interactive manner.
